Job summary

Harmony Hospice in Rhode Island is seeking a compassionate Home Health Aide to join our team. This full-time role provides personal care and supportive services to hospice patients under the supervision of a Registered Nurse, aiming to ensure comfort, safety, and quality of life.

The ideal candidate holds a Rhode Island HHA license, completes at least 75 hours of state training, and maintains a current BLS/CPR certification.

Qualifications

• Must hold a current Home Health Aide license in Rhode Island.
• Complete at least 75 hours of state HHA training (classroom and supervised).
• Current BLS/CPR certification required.
• Prior hospice, home health, or patient care experience preferred.
• Ability to work independently with empathy.
• Valid driver's license and reliable transportation.
• Excellent communication, observation, and documentation skills.

Responsibilities

• Provide personal care and activities of daily living (bathing, grooming, dressing, toileting, mobility).
• Assist with positioning, transfers, ambulation and comfort measures.
• Observe and report changes to supervising nurse.
• Document care and patient observations per hospice policies.
• Promote comfort, safety, dignity, and independence per plan of care.
• Provide emotional support to patients and families.
• Maintain a clean, safe, organized patient environment.
• Collaborate with interdisciplinary hospice team to ensure continuity of care.
• Perform other duties as assigned.

The Home Health Aide provides personal care and supportive services to hospice patients under the supervision of a Registered Nurse, helping ensure comfort, safety, and quality of life throughout their care journey.

Responsibilities for Home Health Aide (Hospice):

• Provide personal care and activities of daily living, including bathing, grooming, dressing, toileting, oral hygiene, and mobility assistance.

• Assist patients with positioning, transfers, ambulation, and other comfort measures as directed by the Registered Nurse.

• Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse.

• Accurately document care provided and patient observations in accordance with hospice policies and regulatory requirements.

• Promote patient comfort, safety, dignity, and independence while following the individualized plan of care.

• Provide compassionate emotional support to patients and their families throughout the hospice journey.

• Maintain a clean, safe, and organized patient environment.

• Collaborate with the interdisciplinary hospice team to ensure continuity of care and exceptional patient outcomes.

• Perform other duties as assigned.

Qualifications for Home Health Aide (Hospice):

• Must hold a current Home Health Aide license in Rhode Island.

• Must meet all state Home Health Aide training requirements, including completion of at least 75 hours of classroom and supervised practical training.

• Current BLS/CPR certification required.

• Previous hospice, home health, or patient care experience preferred.

• Understanding of the hospice philosophy and ability to provide compassionate end-of-life care.

• Ability to work independently with minimal supervision.

• Compassionate, dependable, and emotionally mature with empathy for individuals who are ill, frail, or terminally ill.

• Valid driver's license with reliable, insured transportation required.

• Excellent communication, observation, and documentation skills.
