Job summary

Harmony Hospice in Rhode Island is seeking a compassionate Home Health Aide to join our team. This full-time role provides personal care and supportive services to hospice patients under the supervision of a Registered Nurse, aiming to ensure comfort, safety, and quality of life.

The ideal candidate holds a Rhode Island HHA license, completes at least 75 hours of state training, and maintains a current BLS/CPR certification.

Qualifications

Must hold a current Home Health Aide license in Rhode Island.
Complete at least 75 hours of state HHA training (classroom and supervised).
Current BLS/CPR certification required.
Prior hospice, home health, or patient care experience preferred.
Ability to work independently with empathy.
Valid driver's license and reliable transportation.
Excellent communication, observation, and documentation skills.

Responsibilities

Provide personal care and activities of daily living (bathing, grooming, dressing, toileting, mobility).
Assist with positioning, transfers, ambulation and comfort measures.
Observe and report changes to supervising nurse.
Document care and patient observations per hospice policies.
Promote comfort, safety, dignity, and independence per plan of care.
Provide emotional support to patients and families.
Maintain a clean, safe, organized patient environment.
Collaborate with interdisciplinary hospice team to ensure continuity of care.
Perform other duties as assigned.
