Job summary

Harmony Hospice in Rhode Island is seeking a dedicated Home Health Aide to join our team. This full-time role provides personal care and supportive services to hospice patients under RN supervision, focusing on comfort, safety, and quality of life.

The Home Health Aide will document observations, assist with ADLs, collaborate with the hospice team, and support families throughout the care journey. Qualifications include an active Rhode Island HHA license, BLS/CPR, and ability to work

Qualifications

• Must hold a current Rhode Island Home Health Aide license.
• 75 hours of classroom and supervised practical training.
• Current BLS/CPR certification.
• Previous hospice, home health, or patient care experience preferred.
• Understanding of hospice philosophy and end-of-life care.
• Ability to work independently with minimal supervision.
• Compassionate, dependable and emotionally mature with empathy.
• Valid driver’s license with reliable transportation.
• Excellent communication, observation and documentation skills.

Responsibilities

• Provide personal care and activities of daily living (bathing, grooming, dressing, toileting, oral hygiene, mobility).
• Assist patients with positioning, transfers, ambulation, and other comfort measures as directed by the RN.
• Observe and report changes in the patient’s condition to the supervising nurse.
• Accurately document care provided and patient observations per hospice policies.
• Promote patient comfort, safety, dignity and independence per plan of care.
• Provide compassionate emotional support to patients and families.
• Maintain a clean, safe and organized patient environment.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care.
• Perform other duties as assigned.
