Job summary

The VNA Health Group in Ocean County, NJ is hiring a Home Health Aide to join our Hospice Team. This full-time role provides personal care, mobility assistance, and daily living support for patients in their homes, helping maintain comfort and independence.

Qualified candidates hold a Home Health Aide certification, HS diploma or GED preferred, a valid driver's license, and a reliable car. Strong communication and interpersonal skills are required.

Responsibilities

• Provide personal care services per plan of care.

• Perform similar related tasks as assigned.

Responsibilities:

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Provides personal care services as established in the plan of care
• Assists patients with mobilization and rehabilitation
• Communicates with patients and their families on a regular basis
• Submits required documentation as per agency policy and procedures
• Ensures compliance of agency infection control and safety policies
• Adheres to the organization’s policy in regards to absenteeism and appearance

Qualifications:

Working Conditions/Physical Demand

Working indoors with client/customer contact; use of specific equipment as braces, bedpan, crutches, electrical bed, hoyer lift, personal automobile, walker, wheelchair, household appliances, vacuum cleaner etc.
