Job summary

AdventHealth in Merriam, KS is seeking a Home Health Aide to provide compassionate in-home care across Shawnee Mission-area patients. You will document care, support daily living activities, monitor vital signs, and coordinate with the care team to ensure comfort and dignity in a home setting.

The role requires HS diploma or equivalent, CNA/BLS CPR certification, and healthcare experience. Travel and multi-visit scheduling are essential, with a Day shift and good patient communication skills.

Qualifications

• High school diploma or equivalent required.
• CNA certification and/or healthcare experience preferred.
• BLS/CPR certification required.

Responsibilities

• Provides personal care to patients according to the care plan.
• Documents patient care and observations timely.
• Participates in end-of-life care for patients and families.

Job Description:

Schedule: Full Time Days 8:30am-5pm

Location: Coverage area: Field work covering Shawnee Mission area in their home setting.

• Provides personal care to patients according to the assignment details and care plan.
• Documents patient care and observations timely and appropriately.
• Participates in end-of-life care for patients and families. Informs the registered nurse of any changes in the patient's condition.
• Follows the schedule of care, including making several visits per day with travel.
• Assists patients with activities of daily living, such as bathing, dressing, and grooming.
• Monitors and records vital signs, including temperature, pulse, and respiration.
• Supports patients with mobility and exercise as prescribed. Provides emotional support and companionship to patients. Maintains a clean and safe environment for patients.
• Communicates effectively with patients, families, and healthcare team members.
• Other duties as assigned.
