Hospice of South Texas is seeking a compassionate Registered Nurse to join our Inpatient Center in Victoria, TX. This PRN position provides direct skilled nursing care to hospice patients of all ages and coordinates care with the interdisciplinary team, patients, families, and referring agencies to ensure continuity and quality of care.

Ideal candidates will have strong clinical judgment, excellent communication, and a passion for palliative and end-of-life care.

Formación

• Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing.
• Current Texas Registered Nurse license.
• Valid Texas Driver's License.
• Minimum two years clinical RN experience preferred.
• Hospice or home health experience strongly preferred.

Responsabilidades

• Assess hospice patients to identify physical, psychosocial, and environmental needs
• Develop, implement, and document individualized plans of care
• Provide direct skilled nursing care in accordance with hospice standards
• Coordinate care with the interdisciplinary team and referring providers
• Provide clinical direction to Hospice Aides and LVNs
• Communicate effectively with patients, families, and healthcare professionals
• Promote hospice philosophy, agency policies, and quality standards
• Maintain clinical competency and participate in professional development
