Job summary

Hospice Austin is seeking an RN Case Manager to provide and coordinate professional nursing care within the homecare program in Spicewood/Austin, TX. The role reports to the Team Leader and involves assessment, care planning, and family education in a compassionate hospice setting.

Responsibilities include nursing assessments, symptom management, discharge planning, and collaboration with the IDT to ensure optimal patient and family support.

Qualifications

• Graduation from an approved school of professional nursing.
• Unrestricted Texas RN license.
• Valid Texas driver’s license and safe driving record.
• CPR certification.

Responsibilities

• Provides professional nursing care for hospice patients in the homecare setting.
• Performs initial and ongoing assessments of physical, psychosocial, and environmental needs including pain and vital signs.
• Develops and maintains an individualized plan of care for palliative and end-of-life needs.
• Educates patients and caregivers about disease process, self-care, and end-of-life options; may train staff/caregivers.
• Documents assessments and progress; coordinates with IDT and healthcare providers.
