Job summary

SSM Health is hiring a Certified Nursing Assistant for the Continuum of Care Home Health & Hospice in Wisconsin. This PRN role supports patients and families, performs nursing tasks under RN supervision, and helps with admissions, discharge, and daily care needs.

The position requires CNA certification and a state-approved nurse aide training program. A BLS HCP certification and valid driver’s license are also expected, with MVR checks as part of the routine credentialing process.

Qualifications

• Completion of an approved nurse aide training program and passing grade on state exam.
• No experience required.
• Proof of motor vehicle liability insurance on hire.

Responsibilities

• Receives patient care assignment from the RN and initiates delegated tasks based on competence.
• Collects and documents data for admissions and reassessments for patient groups.
• Maintains safety and quality care by charting and communicating with team.
• Notifies licensed nurse of changes in patient condition or educational needs.
• Assists with daily activities, vital signs, baths, mobility, hygiene, and basic procedures.
• Prepares patients for discharge and accompanies them home if indicated.
• Stocks supplies and assists with clerical duties as needed.
• Orients new patients and family members to the unit.
• Operates equipment per manuals and reports any issues.
