Job summary

AccentCare is hiring in Loveland, CO for a caregiver role to support patients in their homes. You will follow the interdisciplinary plan of care, provide personal care, assist with mobility, meals, and daily activities, and complete required documentation.

The position emphasizes compassionate care and confidentiality, with opportunities for continuing education. The pay range is $17.00 - $22.00 per hour with a $5,000 bonus.

Qualifications

• Follow the interdisciplinary plan of care, be alert to the patient's needs and concerns
• Provide personal care, including bathing, dressing, grooming, hair, nail, skin and foot care and oral hygiene
• Assist patients, according to the plan of care
• Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care
• Observe and report changes in the patient's condition and needs to the Team Director or Registered Nurse
• May perform simple homemaking chores for the patient directly, meal preparation, and/or light housekeeping
• Complete required documentation per policy
• Participate in continuing education programs
• Follow confidentiality regarding patients and families per policy
• Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization
• Attend patient care conferences as requested by the TD
• Perform other related duties and special projects, as assigned
• Perform other duties as assigned including those within the scope of current, valid professional licensure/credentials

Responsibilities

• Follow confidentiality regarding patients and families per policy
• Demonstrate a compassionate attitude toward patients and families
• Attend patient care conferences as requested by the TD
• Perform other duties as assigned

4. Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care


5. Observe and report changes in the patient's condition and needs to the Team Director (TD) or Registered Nurse (RN)


6. May perform simple homemaking chores for the patient directly, meal preparation, and/or light housekeeping in the patient's immediate area


7. Complete required documentation per policy


8. Participate in continuing education programs


9. Follow confidentiality regarding patients and families per policy


10. Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization


11. Attend patient care conferences as requested by the TD


12. Perform other related duties and special projects, as assigned


13. Perform other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.).
