Job summary

Anchor Health is seeking a Hospice Clinical Liaison to grow patient census in Sacramento and surrounding counties. You will build relationships with physicians, hospitals, SNFs, and other partners, and lead outreach events, education, and community engagement.

The role emphasizes proactive, solutions-oriented collaboration across the referral-to-admission process, with a focus on growth and strong partner networks.

Qualifications

• LVN, RN, or MSW required.
• 1–2 years of healthcare marketing/sales experience preferred.
• Minimum of 2 years hospice experience highly preferred.
• Strong familiarity with local referral partners including: Hospitals, Physicians, Board & Care (B&C), Assisted Living Facilities (ALF), RCFE, SNF.
• Excellent written and verbal communication skills.
• Self-directed with the ability to work independently.
• Determined, collaborative, and strong work ethic.
• Consistent, persistent, patient, and compassionate interpersonal approach.
• Valid driver’s license, clean driving record, and current auto insurance required.

Responsibilities

• Build and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners.
• Plan and coordinate marketing events, educational sessions, and community luncheons.
• Educate healthcare stakeholders on hospice and palliative care services.
• Support patients, families, and referral partners throughout the referral and admission process.
• Provide hospice informational / goals of care conversations with patients and families.
• Facilitate initial patient consent process.
• Attend weekly meetings to report successes, share competitor insights, and identify growth opportunities.
• Meet or exceed performance expectations set by the Director of Business Development and Executive Director leadership.
