Job summary

Alamo Hospice is seeking an RN Case Manager to join our growing team in Waco, Texas. This field-based role manages a patient caseload, delivers therapeutic nursing, and coordinates care for patients and families in home settings.

Territory covers Waco, TX and surrounding counties. Schedule is Monday–Friday 8am–5pm with on-call 1-2 weeknights/week and one weekend every 4–5 weeks. Benefit-rich role with a compassionate hospice team.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Manage patient and family caseload and all nursing process phases.
• Assess patient data including physical, psychological, social, and spiritual needs.
• Coordinate care with attending physician and interdisciplinary team.
• Provide regular home visits to reassess needs and update care plans.
• Perform clinical pronouncement of death and support family.
• Maintain accurate patient records and care plans.
• Supervise delivery of care by Home Health Aide per guidelines.
