Job summary

Choice Health At Home is seeking a dedicated Hospice Field Registered Nurse (RN) to provide compassionate care in patients' homes in Fort Worth, TX. This full-time role requires strong clinical expertise and the ability to work independently within an interdisciplinary team.

You will assess needs, develop care plans, educate families, and coordinate with physicians, social workers, and chaplains to ensure comfort and dignity for patients with life-limiting illnesses.

Qualifications

• Current RN License.
• CPR Certification.
• Valid Driver’s License.
• Valid Auto Insurance.
• At least 2 years of Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients’ physical, functional, psychosocial, and environmental needs.
• Determine eligibility and level of hospice care based on criteria.
• Develop, document, and update the plan of care to ensure quality and continuity of care.
• Educate patients and families about disease progression, self-care, nutrition, and end-of-life options.
• Coordinate with the IDG to address needs and provide caregiver guidance.
