Job summary

Catholic Hospice is seeking a Registered Nurse for home-based hospice care in South Florida. The RN will assess patients, develop and implement individualized care plans, administer medications, and educate families within the interdisciplinary team framework.

Requires Florida RN licensure, 1–2 years of nursing experience, hospice experience preferred, and English proficiency. Availability for travel across South Florida may be required, with adherence to hospice standards and regulations.

Qualifications

• Active registered nurse license to practice in Florida.
• 1–2 years of nursing experience preferred.
• Hospice nursing experience preferred.
• Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
• Must be able to read, write, and understand English.

Responsibilities

• Plan and deliver patient care using the nursing process in home settings.
• Develop and update an individualized plan of care with measurable goals.
• Administer medications and treatments as prescribed by physicians.
• Educate patients and families and coordinate with the Interdisciplinary Team.
• Document assessments and interventions accurately in the electronic medical record.
