Job Description

Purpose of Job: To provide nursing care, support, and education to terminally ill patients and their families to maximize comfort, respecting the patient's chronological age, developmental age, or social maturity.

Essential Functions

• Promote patient safety by adhering to policies, procedures, guidelines, Joint Commission standards, and state and Medicare regulations. Complete HIS as per policy (90%). Ensure HR and Occupational Health requirements are completed on time (100%), including licensure, RAHE, flu shots, etc.
• Demonstrate excellent clinical competency according to departmental, program, and discipline standards. Complete Home Health Aide supervision (100%).
• Assess, plan, implement, and document patient care needs to achieve quality outcomes, demonstrating patient case management skills.
• Communicate and collaborate with patients, families, physicians, and others for care coordination and customer service, maintaining a documentation compliance of 90%.
• Manage costs and enhance revenue to demonstrate fiscal responsibility.
• Limit special requests for DME/Supplies.
• Complete authorizations and discharges according to policy (90%).
• Perform on-call duties as necessary to meet patient needs (10%).

Qualifications

• Registered Professional Nurse with a current license from the Bureau of Professional & Occupational Affairs in Pennsylvania.
• One year of recent medical-surgical or community health experience preferred; two years preferred.
• Current CPR certification.
• Valid driver's license, auto insurance, and ability to drive in inclement weather. Auto in good condition. Professional attire required. Basic computer skills.

Hospice nursing staff must be available for on-call as needed and reside within 30 minutes of the hospital.
