Job summary

Halifax Health’s hospice program in Port Orange, FL is seeking a Field Certified Nursing Assistant to provide compassionate personal care and light housekeeping for hospice patients across homes, nursing facilities, and other settings. You will work with an Interdisciplinary Team to follow care plans and ensure comfort and dignity for patients and families.

Qualifications

• High School Diploma or GED equivalent.
• Current Florida CNA certification required.
• Valid driver's license with a clean driving record.
• Ability to work well with Interdisciplinary Team Members.
• Excellent interpersonal and verbal communication skills.
• Excellent customer service skills to maintain positive relationships with patients, families and clinicians.

Responsibilities

• Provide personal care and light housekeeping to hospice patients.
• Assist skilled nursing staff by providing support to caregivers.
• Complete daily schedule of visits per plan of care developed by the Interdisciplinary Team.
