Job summary

The Field CNA at Halifax Health provides personal care and light housekeeping to hospice patients across homes, nursing facilities, ALFs and other settings, following a plan of care developed by the Interdisciplinary Team. You will support skilled nursing staff and complete the daily visit schedule across diverse environments.

A valid Florida CNA certification, high school diploma, and a reliable vehicle are required to perform effectively in this role while maintaining professional

Qualifications

• High School Diploma or GED is required.
• Current/active Florida CNA certification required.
• Valid driver’s license with a clean driving record; personal car in good working order.
• Able to work well with Interdisciplinary Team Members.
• Excellent interpersonal and verbal communication skills.
• Excellent customer service skills to maintain positive relationships with patients, families and clinicians.
• Ability to cope with high stress levels related to terminally ill patients and adapt to changing assignments.

Responsibilities

• The Field CNA provides personal care and light housekeeping tasks to hospice patients in various settings per plan of care.
• Assists skilled nursing staff by providing personal care training and support to the caregiver.
• Responsible for completing the daily schedule of visits.
