The Access Center RN is a field-based position responsible for performing initial physical, psychosocial, and health systems assessments to develop a care plan, set goals, and make recommendations based on patients’ needs. Assessments may occur in patients’ homes, hospitals, or nursing facilities. The RN will follow standards of practice established by the National Hospice Organization when admitting patients and ensure the admission process complies with current regulations. The RN will also engage in community outreach to physicians and other medical providers to share information regarding hospice and palliative care, and gather feedback to inform performance improvement initiatives. Must be able to cover Queens & Nassau.
