Job Description: Access Center RN

When days cannot be added to a life, we add life to remaining days. Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities, and hospitals.

The Access Center RN is a field-based position responsible for performing initial physical, psychosocial, and health systems assessments to develop a care plan, set goals, and make recommendations based on patients’ needs. Assessments may occur in patients’ homes, hospitals, or nursing facilities. The RN will adhere to standards of practice established by the National Hospice Organization when admitting patients and ensure the admission process complies with current regulations. The RN will also engage in community outreach to physicians and medical providers to share information about Hospice and Palliative care and gather feedback for performance improvement initiatives. Must be able to cover Brooklyn & Queens.

Qualifications:

1. Graduate from an accredited School of Nursing; BSN preferred.
2. Minimum of two years experience in Medical-Surgical or Community Health; hospice or oncology nursing preferred.
3. Proficiency in Word and Excel.
4. Valid New York State RN licensure.
5. CPR-BLS certification and a valid NYS Driver's license preferred.
6. Effective communication skills in a customer service environment.
7. Flexibility to work hours as needed.
8. Ability to coordinate resources and personnel within an interdisciplinary team.
9. Strong teaching, communication, and listening skills.
10. Ability to work non-judgmentally with patients and families from diverse cultural, religious, socioeconomic backgrounds, or lifestyles.
11. Proficiency in inputting clinical data.
12. High degree of accountability.
