A leading healthcare provider is seeking an Access Center RN to join their multidisciplinary team. This field-based role involves conducting assessments and developing care plans for patients in various settings. The RN will also engage with the community to promote Hospice and Palliative care services. Ideal candidates will have a BSN, strong communication skills, and experience in Medical-Surgical or Community Health nursing.
When days cannot be added to a life, we add life to remaining days. Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities, and hospitals.
The Access Center RN is a field-based position responsible for performing initial physical, psychosocial, and health systems assessments to develop a care plan, set goals, and make recommendations based on patients’ needs. Assessments may occur in patients’ homes, hospitals, or nursing facilities. The RN will adhere to standards of practice established by the National Hospice Organization when admitting patients and ensure the admission process complies with current regulations. The RN will also engage in community outreach to physicians and medical providers to share information about Hospice and Palliative care and gather feedback for performance improvement initiatives. Must be able to cover Brooklyn & Queens.
