Responsibilities

• Provide a full range of advanced clinical home care services, including patient assessment, medication management, and patient/family education.
• Coordinate, implement, and evaluate the ongoing home care needs of your patients.
• Review patient insurance information, coordinate care plans, and share pertinent findings with family and professional caregivers.

Minimum Qualifications

• Excellent clinical assessment skills.
• Ability to solve problems independently and collaborate with an integrated team.
• Current NYS RN license and registration.
• Bachelor's degree in nursing preferred.
• Minimum of one year of acute medical-surgical nursing experience.
• CHHA experience preferred.
