Job summary

Halcyon Home LLC in Cypress, Texas, invites a compassionate RN with hospice leadership to become the Hospice Director of Nursing (DON). Lead clinical departments, ensure high-quality patient care, and support a growing geriatric hospice population in Tomball and surrounding areas.

The ideal candidate holds a BSN (MSN preferred), Texas RN license, and a proven track record in hospice leadership, customer service, and team development. Excellent PTO, holidays, and team support are offered.

Qualifications

Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing (MSN) preferred.
Current Texas RN license or as per Board of Nursing.
Minimum of 2 years nursing experience with at least 1 year in a leadership role in hospice/palliative care.
Strong understanding of hospice regulations and standards.
Home Care Home Base experience strongly preferred.

Responsibilities

Directs and coordinates clinical departments; ensures continuity, quality, and safety of services in compliance with state and federal regulations.
Promotes hospice philosophy and administrative policies to ensure quality patient care.
Supervises staff to ensure quality, adherence to Plan of Care, and continuity of services.
Monitors the QAPI program and implements corrective actions as needed.
Educates about infection control and standard precautions in patient care activities.
Leads interdisciplinary team meetings and participates in IDG/T planning and QAPI activities.
