Job summary

Concho Hearts Hospice is seeking an experienced Director of Nursing (DON) to lead its Freestone County clinical team. The DON will provide clinical leadership, oversee daily operations, mentor RNs, LVNs, and aides, and ensure care aligns with physician orders and CMS standards.

The role emphasizes collaboration with physicians and leadership, staff development, and a patient-centered approach to high-quality hospice care across Fairfield and surrounding areas.

Qualifications

• Active Texas RN license required.
• Hospice nursing experience required.
• Clinical leadership or nursing management preferred.
• Strong knowledge of hospice regulations and documentation.

Responsibilities

• Provide clinical leadership and support to nursing staff and IDT members.
• Oversee daily clinical operations to ensure high-quality hospice care.
• Supervise RNs, LVNs, Hospice Aides and other clinical staff.
• Ensure care complies with physician orders and plans of care.
• Monitor caseloads, visits, scheduling, and staffing needs.
• Ensure timely clinical documentation and regulatory compliance.
• Coordinate with physicians and leadership; participate in IDT meetings.
• Assist with hiring, onboarding, education, and competency evaluations.
• Promote accountability and a positive work environment.
