Job summary

Fortis Health is seeking an experienced Hospice Director of Nursing (DON) to lead clinical nursing services across our hospice agency. You will oversee all hospice clinical operations, ensure compliance with regulations, and provide leadership and development to clinical staff.

You will monitor performance, maintain high-quality patient-centered care, and support accountability across the organization. The role requires RN licensure, Nursing degree, and extensive hospice/home health leadership

Qualifications

• RN license required in the state of practice.
• Bachelor’s degree in Nursing or a related field required; BSN preferred.
• Minimum of 1 year of management or supervisory experience in a clinical setting.
• Minimum of 2 years of Director of Nursing experience in hospice or home health.
• Minimum of 5 years of hospice clinical experience.

Responsibilities

• Lead and oversee all hospice clinical operations and nursing staff.
• Ensure compliance with federal, state, and accrediting regulations.
• Manage staffing, performance evaluations, training, and staff development.
• Oversee patient assessments, plans of care, documentation, and discharge planning.
• Ensure coordination of care across all disciplines.
• Monitor quality outcomes, patient safety, and infection control practices.
• Address and resolve patient, family, and agency concerns.
• Develop, implement, and maintain clinical policies and procedures.
