Apploi in Chicopee, MA is seeking an experienced Hospice Director of Patient Care Services to lead a multi-disciplinary clinical team and ensure high-quality, compliant patient care. This full-time, on-site role offers a significant leadership opportunity with impact on patient families across Western Massachusetts.

The ideal candidate will hold an active MA RN license (BSN preferred) with 3+ years of hospice or home health leadership, strong Medicare CoPs knowledge, and experience with EMR

Qualifications

• Active Massachusetts RN license (BSN preferred).
• 3+ years of hospice or home health clinical leadership.
• Strong knowledge of Medicare hospice CoPs.
• Experience with EMR systems and survey preparation.
• Compassionate, organized and team-focused leader.

Hospice Director of Patient Care Services - Chicopee, MA

We are hiring an experienced Hospice Director of Patient Care Services for a growing hospice program serving patients and families in Western Massachusetts. This is a full-time leadership role with real impact.

Responsibilities

• Direct the clinical team of RNs, LPNs, aides, social workers and chaplains

• Ensure quality, compliance and survey readiness under Medicare hospice CoPs

• Oversee admissions, plans of care, IDG meetings and clinical documentation

• Manage staffing, on-call coverage, productivity and budgets

• Recruit, train and mentor clinical staff and lead QAPI

Qualifications
