SNFout in Chicopee, MA is seeking an experienced Hospice Director of Patient Care Services to lead the clinical team and ensure high-quality care in Western Massachusetts.

This full-time, on-site role requires MA RN license, hospice leadership experience, Medicare CoPs knowledge, and EMR proficiency.

You will oversee admissions, plans of care, IDG meetings, budgeting, staffing, and QAPI while mentoring staff.

Qualifications

• Active Massachusetts RN license (BSN preferred).
• 3+ years of hospice or home health clinical leadership.
• Strong knowledge of Medicare hospice CoPs.
• Experience with EMR systems and survey preparation.

Hospice Director of Patient Care Services - Chicopee, MA

We are hiring an experienced Hospice Director of Patient Care Services for a growing hospice program serving patients and families in Western Massachusetts. This is a full-time leadership role with real impact.

Responsibilities

• Direct the clinical team of RNs, LPNs, aides, social workers and chaplains
• Ensure quality, compliance and survey readiness under Medicare hospice CoPs
• Oversee admissions, plans of care, IDG meetings and clinical documentation

Qualifications
