Duties

• Develop, implement, and evaluate individualized care plans tailored to each patient's needs, ensuring comprehensive symptom management and quality of life improvements.
• Monitor patient progress through regular review of medical records, documentation accuracy, and compliance with HIPAA privacy regulations.
• Oversee shift management to ensure adequate staffing coverage across all hours of operation while maintaining optimal patient monitoring and safety protocols.

Requirements

• Proven experience in critical care environments such as hospice care, home health, assisted living facilities, or acute care settings.

• Familiarity with Medicare regulations related to hospice services and reimbursement processes.
