Job summary

Prime Home Developmental Disabilities Services, Inc. is seeking an experienced Director of Nursing to lead our Home Health and Hospice clinical teams.

The DON will oversee clinical operations, ensure regulatory compliance, and drive agency growth while maintaining compassionate patient care. The ideal candidate holds an active RN license (BSN preferred) and brings at least three years of leadership in home health or hospice.

Qualifications

• Active RN license in good standing.
• BSN preferred.
• Minimum 3 years leadership experience in Home Health, Hospice, or related healthcare setting.
• Strong knowledge of Medicare, Medicaid, and regulatory compliance.
• Excellent leadership, communication, and organizational skills.
• Experience with EMR systems and clinical operations.

Responsibilities

• Oversee clinical operations for Home Health and Hospice services.
• Ensure compliance with Medicare, Medicaid, state regulations, and agency policies.
• Supervise, mentor, and develop clinical staff.
• Monitor quality outcomes, documentation, and patient satisfaction.
• Oversee admissions, referrals, care coordination, and physician order compliance.
• Lead quality improvement initiatives and regulatory survey readiness.
• Build relationships with physicians, hospitals, facilities, and community partners.
• Collaborate with leadership to support census growth and operational goals.
