Essential Functions
- Develop, plan, organize, and direct patient care services in accordance with regulations and standards.
- Implement policies, procedures, goals, and standards relevant to patient services and staff functions.
- Maintain staffing levels to meet patient care demands and ensure proper coverage.
- Support a culture of safety and quality.
- Direct and support QAPI activities, including evaluation, planning, and implementation.
- Assist in financial planning and budget management.
- Hire, orient, train, evaluate, and discipline staff to retain quality personnel.
- Maintain knowledge of Medicare, Medicaid, and third-party payer requirements for reimbursement.
- Collaborate with the Executive Director to ensure quality care, operational efficiency, and business development.
- Coordinate with physicians, payers, patients, families, and community resources to promote and coordinate care.
- Stay updated on clinical methods, trends, and research, communicating updates to staff.
- Support policy and procedural changes within the agency.
- Consult with stakeholders to develop solutions and address issues.
- Set strategic and operational goals, analyze metrics, and implement action plans.
Knowledge, Skills, and Abilities
- Understanding of hospice regulations and standards.
- Knowledge of hospice functions, patient care, and pain management.
- Compassionate approach towards end-of-life issues.
- Flexibility, organization, and ability to work under stress.
- Effective communication skills in English.
- Interpersonal and presentation skills.
- Knowledge of state-specific healthcare modalities and scope of practice.
- Self-directed work ethic with minimal supervision.
- Valid driver’s license, reliable vehicle, and insurance.
- Basic computer skills.
- Leadership in planning, coordinating, and supervising healthcare services.
- Competency in nursing theory, techniques, and community health.
- Understanding of HIS data collection and reporting.
- Focus on patient-centered, value-based care, and continuous improvement.
Education and Experience
- Licensed physician, physical therapist, speech-language pathologist, occupational therapist, or registered nurse.
- Preferably a Bachelor’s Degree in Nursing from an accredited program.
- Minimum of 3 years nursing experience, with preference for community health, home health, or hospice experience in the past five years.
- At least 1 year supervisory and administrative experience.
- Current license from the state Board of Registered Nursing.
- Current CPR certification.
