Qualifications

• Must hold NY State license or permit in creative arts therapy.
• Board certification in music therapy (CBMT) is required.
• Minimum of one year experience in hospice, palliative care, or acute medical settings.
• Valid driver license and access to a car.

Responsibilities

• Effectively accepts, triages, and follows-up on referrals for creative arts therapy services.
• Follows professional and regulatory visit procedures for home and facility-based health care, including office-based counseling and groups.
• Provides a comprehensive clinical assessment at the initial visit, and updates accordingly.
• Executes a variety of creative arts therapy interventions to meet the client’s needs.
• Provides clinical documentation according to agency guidelines and medical record standards.
• Promotes the mission and growth of the organization and the creative arts therapy program.
