Job summary

ProCare Hospice of Nevada is seeking a Community Liaison to develop and execute networking and marketing strategies aimed at community and referral sources to generate qualified referrals. The role promotes hospice benefits and maintains a credible community image for ProCare.

The Community Liaison will manage CRM data, travel to communities to establish relationships, and collaborate with Admissions to support timely patient admissions. A background in sales and healthcare settings is preferred.

Qualifications

• Must be a sales professional with proven relationship-building skills.
• One year of experience in hospice or health services preferred.
• Excellent verbal and written communication abilities required.
• Must be flexible for evenings and weekends.
• Background check, pre-employment drug screen, TB testing and references required.
• Read, speak, write, and understand English.
• Must have reliable transportation and valid driver’s license.
• CPR/BLS certification required.

Responsibilities

• Foster relationships with community and referral sources.
• Promote hospice benefits and ProCare policies to the community.
• Maintain accurate CRM records and track visits.
• Plan monthly goals for referrals and admissions.
• Coordinate community education events and outreach activities.
• Work with Admissions Coordinator to ensure timely patient admissions.
• Travel within territory to meet prospects and healthcare professionals.
• Adhere to agency regulations and company policies.
