About The Role

The Community Liaison serves as a key ambassador for Unified Care Hospice in the Las Vegas area. In this role, you will develop and nurture relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners. You will work closely with our clinical and leadership teams to ensure referrals are supported with excellent communication, responsiveness, and care coordination.
