Job summary

Community Healthcare of Texas (CHOT) in Fort Worth, TX, is seeking a Community Marketing Liaison to promote hospice services to physicians and discharge planners. You will serve as the bridge between the hospice program and the medical community, driving referrals and visibility.

Qualifications

• College degree or equivalent experience.
• 2+ years of marketing experience.
• Hospice or Home Health experience.
• Proficient computer skills.
• Ability to build relationships and communicate clearly.

Responsibilities

• Generate referrals from physicians, discharge planners, and hospital staff to promote hospice.
• Maintain community visibility and attend networking programs.
• Coordinate education programs in hospitals, physicians' offices, and communities.
• Track and report monthly contacts with referral sources.
• Participate in weekly team conferences with required information and documentation.
• Assist with events, health fairs, and public relations activities.
• Assist with admission process by meeting patients and completing documentation.
