Job summary

LilyCare Hospice of New Mexico is seeking a Hospice Community Liaison to grow our referral network across the greater Albuquerque area. You will visit hospitals, SNFs, assisted living communities, and physician practices, initiating regular sales calls and community education events.

The role emphasizes autonomy with real playbook control over territory strategy, budget management, and collaboration with our clinical team to ensure clean referral handoffs.

Qualifications

• 2+ years of hospice experience (preferred).
• Valid driver's license and reliable transportation for daily travel.
• Established referral relationships in the Albuquerque area preferred.
• Persuasive, outgoing communicator.

Responsibilities

• Own the greater Albuquerque territory as our Hospice Community Liaison and build referral relationships with hospitals, facilities, and physician practices.
• Execute weekly, monthly, and quarterly territory strategy to grow market share and meet admission goals.
• Plan and deliver in-services and community education events about hospice referrals.
• Manage the marketing budget and project return on spend.
• Document activity in the CRM and report progress at staff meetings.

We are hiring a Hospice Community Liaison to grow our referral network and bring our care to more families across the metro.

Duties and Responsibilities

• Own the greater Albuquerque territory as our Hospice Community Liaison — build and deepen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices through consistent, well-planned sales calls
• Execute a weekly, monthly, and quarterly territory strategy — prospecting, account development, and call routing — to grow market share and meet admission goals
• Plan and deliver in-services and community education events that show medical and community groups what hospice offers and when to refer
• Manage your marketing budget like it's your own money, projecting return on what you spend
• Document activity in the CRM, report account progress at staff meetings, and partner with our clinical team on clean referral handoffs

Compensation and Schedule

The Hospice Community Liaison role pays $82,000–$87,000 per year plus performance bonus. Full-time, Monday–Friday, with occasional evenings and weekends for community events and referral-source availability. Territory is the greater Albuquerque area, based out of our office at 4811 Hardware Dr NE, Albuquerque, NM.
