Job summary

Miracle City Hospice in Ashtabula, OH is seeking a compassionate CNA/STNA to join our hospice team and provide comfort, dignity, and support to patients and families in home care settings.

You will play a vital role in helping patients remain comfortable and maintain their dignity while receiving care at home. The team offers comprehensive benefits and a collaborative work environment.

Qualifications

• Current CNA or STNA certification in Ohio.
• Current CPR certification.
• Active status on the state aide registry with a clear record.
• Ability to work independently while collaborating with a care team.
• Strong communication and organizational skills.

Responsibilities

• Provide personal care and assistance with activities of daily living (bathing, grooming, dressing, oral hygiene).
• Support patient comfort and safety per individualized plans of care.
• Assist with mobility, transfers, ambulation, and prescribed exercises.
• Monitor and report changes to the RN Case Manager.
• Record vital signs and document patient care accurately.
• Assist with meal preparation, feeding, and light tasks as assigned.
• Provide emotional support and companionship to patients and families.
• Maintain a clean, safe, and respectful home environment.
• Offer respite support to caregivers when appropriate.
• Collaborate with the hospice team to ensure exceptional patient care.

Schedule: Monday-Friday, 8:00 AM - 5:00 PM

What You'll Do

As a Hospice CNA/STNA, you'll play a vital role in helping patients remain comfortable and maintain their dignity while receiving care in their homes.

Key responsibilities include:

• Providing personal care and assisting with activities of daily living, including bathing, grooming, dressing, and oral hygiene
• Supporting patient comfort and safety by following individualized plans of care
• Assisting with mobility, transfers, ambulation, and prescribed exercises
• Monitoring and reporting changes in patient condition to the RN Case Manager
• Recording vital signs and documenting patient care accurately
• Assisting with meal preparation, feeding, and light patient-related tasks as assigned
• Providing emotional support and companionship to patients and their families
• Helping maintain a clean, safe, and respectful environment
• Offering respite support to caregivers when appropriate
• Working closely with the interdisciplinary hospice team to ensure exceptional patient care

What We're Looking For

• Current CNA certification or STNA certification in Ohio
• Current CPR certification
• Active status on the state aide registry with a clear record
• Ability to work independently while remaining connected to a collaborative care team
• Strong communication and organizational skills
• Compassionate, empathetic, and patient-focused approach to care
• Comfortable supporting patients and families through the end-of-life journey
• Valid driver's license and reliable transportation with insurance as required by state regulations
