Job summary

Syringa Hospital & Clinics in Idaho seeks a Hospice Nursing Assistant to support licensed nurses by delivering basic patient care to hospice patients. The role involves assisting with ADLs, providing comfort, documenting observations, and helping maintain a clean, safe home-like environment.

You will participate in interdisciplinary care planning, report changes promptly, and engage with patients and families.

Qualifications

• High school graduate preferred.
• Completion of state approved nursing assistant training program/current Board of Registry status required.
• Completed and cleared Idaho state background check.
• Current BLS certification.

Responsibilities

• Assist hospice patients with activities of daily living (ADLs) such as hygiene, grooming, bathing, feeding, transfers and ambulation; maintain a clean, safe home environment.
• Report and record tasks performed and any changes in patient condition.
• Relate well with patients, families, and coworkers; participate in interdisciplinary team meetings.
• Obtain and record data (vital signs, pain levels, catheter care) and report findings per assignment sheet.
• Contribute to care plans and participate in required staff development activities.

Augments the service of the licensed nurse by providing basic patient care services to the hospice patient.

• Assists in the care of the hospice patient including activities of daily living (ADLs) such as, but not limited to, personal hygiene and grooming, bathing, feeding, repositioning, toileting, transfers and ambulation; household services essential to promote a clean, safe environment.
• Accountable for reporting and recording tasks performed as well as any noticed change in the patients condition.
• Must be able to relate well/ work closely with patients, their families and coworkers.
• Obtain and record patient data such as, but not limited to, vital signs, catheter care, pain levels, Report findings outside parameters as noted on the assignment sheet or urgent/emergent conditions. Completes appropriate records as required.
• Participate in interdisciplinary team meetings to discuss and coordinate the patient plan of care.
• Requires creativity and ability to problem solve in the home environment.
• Participate in required staff development activities and performance reviews.

Requirements

• High school graduate preferred.
• Completion of state approved nursing assistant training program/current Board of Registry status required.
• Completed and cleared Idaho state background check.
• Current BLS certification.
• A minimum of 12 hours of hospice-specific training annually is required to continue employment in the hospice department.
