Job summary

Syringa Hospital & Clinics is seeking a compassionate Nursing Assistant to support hospice patients in home-based care. You will provide basic care, assist with ADLs, monitor vital signs, and report changes in patient condition. Strong communication with patients, families, and the care team is essential. Current Idaho CNA registration and BLS are required.

Qualifications

• High school diploma or equivalent preferred.
• Completion of state-approved nursing assistant training program/registry status required.
• Idaho state background check completed and cleared.
• Current BLS certification.

Responsibilities

• Assist hospice patients with activities of daily living (ADLs) such as personal hygiene, grooming, bathing, feeding, transfers, and ambulation.
• Report and document tasks and any changes in patient condition.
• Relate well with patients, families, and coworkers within the care team.
• Record patient data including vital signs, pain levels, and catheter care as required.
• Participate in interdisciplinary team meetings to coordinate the plan of care.
• Demonstrate creativity and problem-solving in the home environment.

Description

Augments the service of the licensed nurse by providing basic patient care services to the hospice patient.

• Assists in the care of the hospice patient including activities of daily living (ADLs) such as, but not limited to, personal hygiene and grooming, bathing, feeding, repositioning, toileting, transfers and ambulation; household services essential to promote a clean, safe environment
• Accountable for reporting and recording tasks performed as well as any noticed change in the patients condition.
• Must be able to relate well/ work closely with patients, their families and coworkers.
• Obtain and record patient data such as, but not limited to, vital signs, catheter care, pain levels, Report findings outside parameters as noted on the assignment sheet or urgent/emergent conditions. Completes appropriate records as required.
• Participate in interdisciplinary team meetings to discuss and coordinate the patient plan of care.
• Requires creativity and ability to problem solve in the home environment.

Requirements

• High school graduate preferred.
• Completion of state approved nursing assistant training program/current Board of Registry status required.
• Completed and cleared Idaho state background check.
• Current BLS certification.
