Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to provide compassionate, hands-on end-of-life care under supervision of the nursing team. Schedule is 7 am to 3:30 pm with coverage area including Crystal Lake, McHenry, Algonquin, Lake Zurich and Fox Lake.

You will follow care plans, assist with personal care, document accurately, and support patients' families while maintaining dignity and comfort.

Qualifications

• High School Diploma or GED required.
• Holds current unencumbered C.N.A. certificate in the state.
• Minimum 1 year experience as a CNA in acute or long-term care; hospice exp a plus.
• CPR certification if state-mandated
• Driver's license, vehicle insurance, and reliable transportation

Responsibilities

• Provide Personalized Care following the care plan and attend promptly to patient requests.
• Assist with personal care including bathing, oral hygiene, changing bedding, clothing, and shampooing.
• Be dedicated to customer service to ensure patient satisfaction.
• Provide support for families by guiding and reassuring them.
• Maintain accurate documentation in patient homes and agency records
