Moments Hospice is seeking a dedicated CNA for an on-the-road role. You will perform daily living activities, support patient comfort, and document in the electronic medical record under a Case Manager RN. A valid CNA license and caregiving experience are required, with CPR certification preferred.

The position includes a company car with fuel and insurance coverage, a cell phone, and comprehensive health benefits. Competitive salary with generous PTO and 401(k) with company match are offered.

Formación

• CNA license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing, Hospital or Hospice.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication and active listening abilities.
• Ability to work in an electronic medical record system.
• Valid driver's license and auto insurance.

Responsabilidades

• Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans and assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition; maintain EMR records.
• Work under the direction of a Case Manager RN.
