Job summary

Moments Hospice is seeking a compassionate CNA to support patient care on the road. You will perform daily living activities, review care plans, assist with transfers, ROM, mobility, dining, and nutrition while keeping records in the EMR.

Work closely with a dedicated Case Manager RN to deliver patient comfort and dignity. The role offers a competitive hourly wage, benefits including health/dental/vision, PTO, a company car with fuel and insurance, and a cell phone allowance.

Qualifications

CNA license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
CPR certification is preferred.
Excellent customer service skills.
Strong communication (written, verbal, active listening).
Problem-solving skills.
Time management skills.
Ability to work in an electronic medical record system.
Ability to lift 50 lbs.
Good understanding of body mechanics.
Strong understanding of professional boundaries.
Valid driver's license.
Valid Auto Insurance.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
