Job summary

The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing team, serving patients in Monticello and surrounding communities within approximately an hour. This role offers days only with no nights, weekends, or on-call, and provides opportunities for career growth including tuition reimbursement for LPN/RN advancement.

You will work with an interdisciplinary team to provide personal care, comfort, and dignity to hospice patients in homes

Qualifications

• Active CNA certification is required.
• Prior hospice/home health/long-term care experience is preferred.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per care plan.
• Observe and report changes in patient condition to supervising nurse.
• Support patients and families with compassion, dignity, and respect.
• Assist with mobility and activities of daily living.
• Document all patient care in the EMR (Homecare Homebase).
