Job summary

The Care Team Hospice in Danville, IL is seeking a compassionate Certified Nursing Assistant (CNA) to join our growing team for the 2nd shift (2p-10p). This role delivers hands-on personal care and comfort to hospice patients in their homes and local facilities, helping families through important moments.

You will work with an interdisciplinary team to maintain dignity, comfort, and quality of life, while documenting care in the Homecare Homebase EMR.

Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience preferred; hospice, home health, or skilled nursing experience encouraged.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per care plan.
• Observe, document, and report changes in patient condition to supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living; maintain a safe environment.
• Complete accurate, timely documentation in the EMR (Homecare Homebase).
