Job summary
The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our team in the Peoria, IL area. This role focuses on hands-on personal care and comfort for hospice patients, supporting families through challenging moments.
As a Hospice CNA you will work with an experienced interdisciplinary team to maintain dignity and quality of life at patients' homes or care facilities. We offer a schedule that avoids nights, weekends, and on-call requirements to support work-life
Qualifications
- Active Certified Nursing Assistant (CNA) certification.
- Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
Responsibilities
- Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per patient care plan.
- Observe, document, and promptly report changes in patient condition to the supervising nurse.
- Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
- Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
- Complete accurate and timely documentation of all patient care in the EMR (Homecare Homebase).
