HopeHealth Hospice and Palliative Care is seeking an experienced Certified Nursing Assistant (CNA) to join our hospice team in Wakefield, RI. This full-time role provides direct patient care in long-term care facilities, under RN supervision, with a consistent 40-hour schedule and rotating weekends.

• Certified Nurse Aide certification required.
• High School Diploma or GED required.
• 1–3 years of experience in home care or long-term care settings required.

Responsibilities

• Provides personal care to hospice residents.
• Observes, reports, and documents patient status and the care provided.
• Provides support to patient, family, and caregiver through ongoing communication.
• Follows basic infection control procedures.
• Reports changes in patient status to RN Case Manager.
• Recognizes emergencies and applies emergency procedures.
• Maintains patient and family confidentiality.
