Job summary

HopeHealth, Inc. seeks an experienced Hospice Certified Nursing Assistant (CNA) to provide compassionate personal care for patients in their homes and in long-term care facilities across Massachusetts, including the Attleboro area.

This full-time role operates 8:00 am–4:30 pm, includes a comprehensive benefits package, and places you on an interdisciplinary hospice team. You will follow the plan of care, observe and report patient status, support families, and uphold dignity, confidentiality,

Qualifications

Certified Nurse Aide certification.
High School Diploma or GED.
1-3 years of experience in home care and long-term care settings required.

Responsibilities

Provides personal care and light housekeeping duties
Observe, report, and document patient status and the care provided
Provide compassionate support to patients, families and caregivers through on-going communication
Follow basic infection control procedures
Report basic elements of body functioning, recognize and report changes in patient's status to RN Case Manager
Recognize emergencies and has the knowledge of emergency procedures and their application
Maintain patient and family confidentiality
