Job summary

Brighton Hospice in Minnesota seeks a Lead CNA to coordinate patient visits, reassign CNAs, and support daily scheduling. This role focuses on leadership, teamwork, and clear communication to ensure high-quality hospice care.

You will collaborate with nursing staff, assist with new-hire orientations, generate weekly reports, and help maintain accurate visit records in the database, contributing to a compassionate, well-organized patient experience.

Qualifications

• Must have previous leadership/management and/or hospice experience.
• Excellent communication skills.
• Prior hospice experience is preferred, but not required.
• Strong familiarity with computer applications and spreadsheets.

Responsibilities

• Receive callouts from CNA field staff and reassign patient visits as needed.
• Ensure daily schedule of patients is complete and updated in database.
• Assist with staffing assignments for new admissions and scheduling as directed by supervisor.
• Respond to phone calls from all disciplines, family members and referral sources.
• Follow up with staff to provide support and education as needed.
• Assist with preparation and facilitation of new-hire orientations for all CNA field staff.
• Complete weekly reports addressed in a weekly meeting with supervisor.

Details

• Lead CNA position that requires previous CNA leadership and/or hospice experience

• Complete weekly reports which will be addressed in a weekly meeting with direct supervisor
• Assist in reviewing and exporting all visits in our database, communicating with CNAs in reference to their documentation and timecards and then reporting this information to supervisor weekly.
