Job summary

Care 360 Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide patient-centered personal care to hospice patients in Toledo, OH and surrounding areas. You will assist with bathing, grooming, toileting, feeding, and mobility, travel to patients’ homes or facilities, and collaborate with the interdisciplinary team to ensure comfort and dignity.

The role requires a valid CNA license in Ohio, CPR/BLS certification, reliable transportation, and willingness to travel.

Qualifications

  • CNA license in the state of practice is required.
  • CPR/BLS certification is required.
  • Hospice experience preferred but not mandatory.

Responsibilities

  • Provide compassionate, patient-centered personal care (bathing, grooming, dressing, toileting, feeding) per the hospice plan of care.
  • Travel to patients' residences, including private homes and facilities, to deliver hospice services.
  • Offer emotional support and companionship to patients and families to promote comfort and dignity.
  • Provide bedside vigil support during end-of-life care as needed.
  • Assist with ambulation, transfers, positioning, and ROM as documented.
