Job summary

Rocky Mountain Care Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide personalized, comfort-focused care to hospice patients in Tooele, UT. This full-time role supports patients and families with dignity and respect in home health settings.

Responsibilities include assisting with ADLs, mobility, and daily tasks, documenting changes, and following each patient’s plan of care. CPR/BLS and a valid driver’s license are required.

Qualifications

• Current CNA certification in good standing.
• CPR/BLS certification as required.
• Valid driver’s license, reliable transportation, and auto insurance.
• Compassionate, dependable, and comfortable working independently.
• Hospice, home health, skilled nursing, or senior care experience is a plus.

Responsibilities

• Assist patients with bathing, grooming, dressing, toileting, and other activities of daily living (ADLs).
• Help with positioning, transfers, mobility, and comfort needs.
• Provide compassionate, one-on-one care in patients' homes or care settings.
• Observe and report changes in patient condition to the hospice nurse.
• Complete required documentation accurately and timely.
• Support patients and families with kindness, dignity, and respect.
• Follow the patient's individualized hospice plan of care.
