Job summary

Qualifications

Responsibilities

• Provides direct personal care including bathing, grooming, and ADL assistance as per patient care plans.
• Assists with mobility, positioning, safety, and transport as needed.
• Documents care rendered within 24 hours with high accuracy and reports changes to RN/Manager.
• Performs light housekeeping in patient homes and supports hospice/POC goals.
• Participates in performance improvement activities and follows supervisory guidelines.
