Job summary

Responsibilities

• Provide personal care and assist with ADLs as per the plan of care.
• Utilize proper body mechanics and assist with positioning and mobility.
• Contribute to patient safety and report concerns to RN/Team Lead.
• Perform light housekeeping in patient homes per hospice plan.
• Complete ~5 billable visits per day and document care within 24 hours with 95% accuracy.
