Job summary

Legacy Lifecare VNA and Hospice is seeking a Hospice Home Health Aide (CNA) to provide compassionate, hands-on care in patients' homes. The role requires current MA CNA certification, at least 1 year of CNA experience, a valid driver’s license, and reliable transportation to travel to patient homes or hospice facilities.

The position offers $20.00–$25.00 per hour, based on experience, with PRN Monday–Friday 8:00AM–4:00PM. Benefits include comprehensive health coverage and strong retirement plans.

Qualifications

• Current, active MA CNA certification.
• Minimum 1 year CNA experience.
• Must have a valid driver’s license.
• Must have reliable transportation and ability to travel to patient homes or hospice facilities.
• Prior experience in home-care, hospice, rehabilitation, or long-term care setting preferred.

Responsibilities

• Provide compassionate, hands-on personal care to hospice patients, including bathing, grooming, dressing, toileting, and mobility assistance.
• Monitor and report changes in patients' condition to the hospice nurse or care team.
• Assist with maintaining a safe, clean, and comfortable home environment for patients and families.
• Offer emotional support and companionship while respecting dignity and end-of-life goals.
