Job summary

Bristol Hospice is seeking a compassionate Certified Home Health Aide (CHHA) to join our interdisciplinary team in Colorado. Under RN supervision, you will provide personal care, assist with daily activities, and support patients and families while maintaining dignity and comfort in their home setting.

Ideal candidates are CNAs in good standing, with strong communication, empathy, and adherence to plan of care. California-specific regulation is noted in the posting where applicable.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Provide positive communication with patients and family within ethical and respectful care.
• Meet safety needs and use equipment safely (e.g., foot stools, side rails, O2).
• Assist in dressing, undressing, meals, feeding, and monitor patient comfort.
• Record vitals as ordered and report significant changes to the Case Manager.
• Assist with transfers, mobility, and routine exercises as directed.
